Form Instructions

Adobe Reader is required to open HCAD Forms. "Fillable" forms may not work properly with versions of Adobe Reader prior to version 7. Visit the Adobe website to download the latest version.

Viewing the Forms

You may view the forms by clicking on the appropriate link.

To save the file to your computer:

1.  Click on the link.
2. Select Select "Save Target As..." or "Save Link As..." 
3. Choose a directory and filename for your PC

Fillable Forms

Fillable forms require a current version of Adobe Reader. To access a form:

1. Right-click on the link.
2. Fill out the form.

ONCE YOU HAVE COMPLETED THE FILLABLE FORM, YOU MUST PRINT IT, SIGN IT, AND MAIL OR DELIVER THE FORM TO:
